By using our website or services, you confirm that you are at least 18 years of age, or if under 18, that you are accessing services with the consent of a parent or legal guardian. You affirm that all information you provide is accurate and complete to the best of your knowledge.
Health Focus Place provides coordinated home health and wellness management services, including general healthcare, dental care, wellness and nutrition programs, hair and skin treatments, joint and bone health, cardio health, men's and women's healthcare, memory and neuro healthcare, and health insurance navigation. Services are available through virtual consultations nationwide and in-person home visits in select service areas.
We offer three membership tiers: Essential Care ($49/month), Comprehensive ($129/month), and Concierge Health ($299/month). Membership fees are billed monthly in advance and are non-refundable except where required by law.
Clinical services such as physician visits, lab tests, imaging, and procedures are billed separately from membership fees. We accept Medicare, Medicaid, and most major private insurers. Self-pay rates are available upon request and are published transparently.
You may cancel your membership at any time with 30 days' written notice. There are no cancellation fees. Cancellation does not affect your responsibility for charges already incurred or services already rendered.
We may adjust our fees with 60 days' prior notice. Existing members will be grandfathered at their current rate for the remainder of their current billing cycle.
The Healthy Life Warranty covers coordination and follow-up care for unexpected complications arising from procedures we coordinate, for up to 12 months following the original procedure. The warranty does not cover complications arising from patient non-compliance, pre-existing conditions not disclosed at intake, or services rendered by providers outside our network without prior authorization. Full warranty terms are provided in your membership agreement.
Telehealth services are provided through secure, HIPAA-compliant video and messaging platforms. You acknowledge that telehealth may not be appropriate for all medical conditions and that our providers will determine, in their clinical judgment, when an in-person visit is necessary. You are responsible for ensuring you have appropriate technology and internet connectivity to participate in telehealth visits.
Information provided on our website, in newsletters, or through our services is for general informational and educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or qualified health provider with any questions regarding a medical condition. To the fullest extent permitted by law, Health Focus Place shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, arising from your use of our services. Our total liability for any claim arising from these terms shall not exceed the amount you have paid us in the 12 months preceding the claim.
You agree to indemnify and hold harmless Health Focus Place, its officers, directors, employees, and agents from any claims, liabilities, damages, losses, or expenses (including reasonable attorney's fees) arising from your use of our services, your violation of these terms, or your infringement of any third-party rights.
We may refer you to third-party healthcare providers, laboratories, pharmacies, or other services. We are not responsible for the quality, safety, or legality of services provided by third parties, and your engagement with them is solely at your own risk.
These terms shall be governed by the laws of the State of Illinois, without regard to conflict-of-law principles. Any dispute arising from these terms shall first be addressed through good-faith negotiation. If unresolved, disputes shall be submitted to binding arbitration in Chicago, Illinois, in accordance with the rules of the American Arbitration Association.
We shall not be liable for any failure or delay in performing our obligations due to causes beyond our reasonable control, including acts of God, natural disasters, war, terrorism, civil unrest, pandemics, government actions, or failures of telecommunications or internet infrastructure.
We reserve the right to modify these Terms and Conditions at any time. We will post the updated terms on this page and, for material changes, notify existing patients by email. Your continued use of our services after the effective date constitutes acceptance of the modified terms.
If any provision of these terms is held to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
These Terms and Conditions, together with our Privacy Policy and your executed membership agreement, constitute the entire agreement between you and Health Focus Place and supersede all prior or contemporaneous agreements, whether written or oral.
